Cantonese/Mandarin: 你好嗎?你好吗?
Czech: jak se máš
Danish: hvordan har du det?
Dutch: hoe gaat het ?, alles goed?
French: comment vas-tu?, ça va?
German: wie geht es dir?
Hebrew: ?מה שלומך
Italian: come stai?
Japanese: お元気ですか?おげんきですか?
Spanish: ¿cómo estás?
Swedish: hur är det?
Ukrainian: як справи?
Vietnamese: bạn khỏe không?

You have several choices when saying "How are you" in Russian.

Как дела? - How's things?
Как ты? - How are you?
Как жизнь? - How's life?

And the most common answer is not the optimistic "Fine". It is a simple:

Нормально. - Normal.
